The Age UK Salford Hospital Aftercare Team who are based in Salford Royal Hospital have been shortlisted for a Staff Northern Care Alliance Award.
There were 626 nominations for individuals and teams across the alliance and the standard of the nominations was very high. It’s a testament to the team that they were shortlisted.
They were recognised for their ongoing hard work throughout the Covid-19 Pandemic including regular welfare calls to support local people.
The Hospital Aftercare Service supports people aged 55 + and their carers for up to six weeks once discharged from hospital. They call people to check they are managing once home and visit them for a personal assessment of health and wellbeing needs.
They often support those who have been in hospital for a longer duration and help them to regain confidence once home to ensure they remain independent, for as long as possible. They also help to assess the home to minimise the risks of falls and trips and support people with basic shopping needs.
